During a medication review we will go over :
- Your health conditions and the medications you take.
- Information about your health conditions.
- The correct way to take your medications.
- Any issues with your medications.
- Changes to your health plan and medications if needed.
- Your questions.
- The need for follow-up.
You should have a medication review if you:
- Want to quit smoking.
- Have diabetes and take at least one prescription medication.
- Have at least one chronic illness and take at least three different prescription medications.
- Have at least two chronic illnesses.
- Have at least one chronic illness and at least one risk factor.
Chronic illnesses:
- High blood pressure
- Diabetes (type 1 or type 2)
- Chronic Obstructive Pulmonary Disease (COPD)
- Asthma
- Heart failure
- Heart disease
Risk factors:
- Obesity
- Addictions
- Tobacco use
- Mental illness
